How To Get Started
1. CONTACT OUR OFFICE
Contact our office to set up 5-10 min consultation to see if this service is right for you. If you need to leave us a message, please make sure you let us know your name and a phone contact number we can reach you at. Please clarify if we may leave a detailed voice message at this number—one of our many priorities is to protect your privacy. You can reach us either by email or by phone:
2. IS TELECBT RIGHT FOR YOU?
One of our staff members will answer any questions you have about the program and help you make a well-informed decision about whether TeleCBT is the right service for you.
3. SUBMIT YOUR SIGNED CONSENT
If you decide to try our services we will need your signed consent. We can either fax, e-mail, or use post-office delivery services to send you our program intake and consent forms. You also have the option to download the forms download the forms online. As soon as you’ve had the chance to review the forms, you may return via e-mail, fax, or post-office delivery services. You may also choose to request to have one of our licensed mental health providers review the forms with you over the phone. Please understand, in an effort to make ourselves available to all our current and prospective patients, we will limit these question and answer sessions to 10 min.
4. SCHEDULE YOUR INTAKE APPOINTMENT
Once we receive your signed consent forms, our administrative coordinator will contact you to schedule your intake appointment with a clinician licensed in your state of residence. Your intake appointment will be approximately 60 minutes. Our coordinator will work with you and your assigned provider to ensure appointments are scheduled at a time that works well with your availability.
5. PURCHASE YOUR SESSIONS
Choose your number of sessions and submit your payment information.
6. HAVE YOUR FIRST TELECBT SESSION
Please expect a call from your assigned TeleCBT provider at the time of your scheduled appointment. Please ensure that you are not driving at the time of your appointment. Our providers put patient safety first and will discontinue your session if you schedule at a time in which your are conducting a vehicle. We treat our teleCBT sessions as we would face-to-face sessions; thus, we will only contact you from a private setting in which we can protect the confidentiality of the information discussed over the phone. We encourage you to also find a private space for you to be able to speak comfortably and without distractions. If you need to reschedule, please call or email our administrative coordinator 48 hours prior to your scheduled appointment time.
PURCHASE YOUR PACKAGE
Care Connected offers a variety of different packages for you to choose from. Whether you want to purchase a single session or multiple sessions, we have an option for you. Please note that purchasing multiple sessions does qualify you for a 10% discount.

Please be sure to print your receipt, so you may submit it to your insurance.
